尊敬的公司領(lǐng)導(dǎo)：
您好!
在這些日子以來，公司給予了我許多機(jī)遇和挑戰(zhàn)，使我累計(jì)了一定的經(jīng)驗(yàn)。同時(shí)也學(xué)到了許多為人處世等做人的道理。所有的這些，我都很珍惜也很感謝您，因?yàn)檫@西都是為我在將來的工作和生活帶來幫助和方便.
現(xiàn)由于個(gè)人的原因，因本人一直以來身體不好，近視度數(shù)不斷加深，就擔(dān)任此份工作之后，經(jīng)常熬夜，睡眠不足，給眼睛帶來的傷害越來越大了，每到上早班的一周里，早上起床眼睛更是疼痛不已，帶著病痛上班，都是靠著忍受度過的。對于由此為公司造成的不便，我深感抱歉。但同時(shí)也希望公司能體恤我的個(gè)人實(shí)際，對我的申請予以考慮并批準(zhǔn)為盼。
